Payments
Health insurance plans are not accepted. Although you may be able to submit receipts for reimbursement, you are considered a “self-pay” patient. Self-pay patients will be responsible for charges associated with service(s) rendered.
Running Late
Please contact the office if you are running late to an appointment. Tardiness to your appointment may require the appointment to be rescheduled. If you arrive more than 10 minutes late for your scheduled appointment, you may be asked to reschedule, and will be charged a $150 fee.
Cancellation/No Show Policy
Your appointments are very important to the us. We understand that sometimes schedule adjustments are necessary; therefore, we respectfully request you provide us 48-hour notice if you need to reschedule or cancel.
Appointments/Consultations that are cancelled less than 48-hours will be charged a non-refundable cancellation fee equal to $150 or 50% of the treatment, whichever is greater. If you do not show up to your appointment and do not call to reschedule, this will result in fees equal to $150 or the full charge of treatment(s), whichever is greater.
