Please plan to spend 45 minutes for your initial consultation. During your visit, Dr. Ephraim will discuss your concerns, perform a thorough history and physical examination and develop your unique treatment plan. Treatments or procedures may be able to be performed the same day or scheduled for a future appointment.
Insurance Plans
We are pleased to accept most major insurance plans for covered medical services. Coverage and benefits vary by plan, and patients are responsible for any applicable copays, deductibles, or non-covered services as determined by their insurance carrier.
Self-Pay / Cash-Pay
For patients who choose not to use insurance or whose services are not covered by insurance, we offer a self-pay consultation option.
• Initial Consultation Fee: $300
• Deposit Requirement: A 30% deposit is required at the time of scheduling to reserve your appointment.
• The deposit is applied toward the cost of your consultation on the day of your visit.
Important Notes
• The remaining balance is due at the time of service.
• Self-pay fees apply to patients who do not wish to bill insurance or whose insurance is not accepted or does not cover the requested service.
• Our team is happy to help verify insurance benefits prior to your visit whenever possible; however, final coverage determinations are made by your insurance provider.
